Requirements to Receive Care
- Patient must be a resident of Brooklyn, Queens or Manhattan
- Patient must be under the care of a physician
- No age requirement
- Patient must reside in a safe home environment
Health Services
- Manage and assist with medications
- Closely monitor your safety and comfort
- Follow plan developed by your doctor
- Registered nurse monitors and supervises care
- Provide physical therapy and rehabilitation care
Personal Care
- Meal preparation, including help with special dietary needs and feeding
- Assist with mobility, including from bed to chair or wheelchair
- Personal grooming help, including hair, teeth, nails
- Assist with bathing and dressing
- Maintain a safe environment in the home
Companion Services
- Accompany to appointments, including doctor visits
- Grocery shopping
- Neighborhood walks with
wheelchair or walker - Socializing
- Light housekeeping
