When the resident is ready to leave the hospital but is not ready to resume independent living, Cobble Hill Health Center’s subacute unit is the bridge to complete health. After admission, residents are evaluated by a physician and other members of the subacute team.
A comprehensive care plan is designed to ensure a quick and complete recovery, incorporating rehabilitation, psychological support and education for the resident and family. Nursing and medical care are outstanding and the care plan is reviewed and updated regularly.
Our goal is to restore our residents’ independence and help them return to community living. Access to the latest medical interventions and a skilled nursing staff help that happen.
While you are at Cobble Hill Health Center, caring social workers create discharge plans and offer ongoing support and counseling.
When the time comes to return home, our social work team will assist in arranging the services you need at home, such as home health aides, adaptive equipment and care management. Through the comprehensive services we offer through Cobble Hill Lifecare, including Your Choice at Home, our certified home health agency, and LifeCare Plus, our care coordination program, our discharge team can ensure that you will have a seamless transition back to your home.

At Cobble Hill Health Center’s Rehabilitation Institute, our goal is to restore residents’ function and the abilities they had prior to the onset of illness, injury or trauma. Providing skillful and compassionate care is our main objective.
Facilitating Admissions
The courteous and streamlined admission process at Cobble Hill Health Center guarantees that within 24 hours of referral, residents can be resting comfortably in their room. With a full-time nurse on duty for evaluation and an experienced financial coordinator familiar with Medicaid, Medicare and many large HMO’s, a resident can be admitted to Cobble Hill’s Rehabilitation Institute quickly and efficiently, even on weekends and evenings. Medicaid applications can be processed and on-site resident assessments can be arranged.
The Best Part is Going Home
Our interdisciplinary team recommendations and in-facility assessments by home-care nurses prior to discharge, ensure safe and timely discharges to home care and the community. Adaptive devices, such as walkers, commodes, wheelchairs and canes are provided. Social workers develop strong lines of communication with families and follow-up once residents are released.
Through the comprehensive services we offer through Cobble Hill Lifecare, including Your Choice at Home, our certified home health agency, LifeCare Plus, and our care coordination program, our discharge team can ensure that you will have a seamless transition back to your home.
Cardiac rehabilitation at Cobble Hill Health Center is designed to meet the physical, emotional, and educational needs of people recovering from a cardiac condition. The ultimate goal of cardiac rehab is to return the resident to an active, healthier life through exercise and lifestyle changes.
What to Expect
A personal exercise program will be developed based on the resident’s medical needs, abilities, and goals. The exercise classes are fun, safely challenging and well-structured. A rehabilitation therapist will closely monitor the resident’s vital signs while exercising using a treadmill, upper and lower body ergosizers, stair-climbing exercise machine and free weights.
Everyone with heart disease, regardless of age, sex or race can benefit from some type of cardiac rehab. No one is too old or too young.

Our Palliative Care Program is a philosophy of care that accepts dying as a natural part of life. When death is inevitable, palliative care seeks neither to hasten nor postpone it.
At the center of palliative care is the belief that each of us has the right to die pain-free and with dignity, and that our families will receive the necessary support to allow us to do so.
We offer a Palliative Care Program designed to ease the outcome of a terminal diagnosis by providing relief from distressing symptoms, by providing pain management therapy and by focused treatment so the resident is kept as comfortable as possible. An interdisciplinary team gives residents and families living with a life-threatening or terminal illness, care that is focused on alleviating suffering and promoting quality of life. Major concerns are pain and symptom management, information sharing and advance care planning, psychosocial and spiritual support, and coordination of care.
At Cobble Hill, the resident’s needs are assessed and all treatment options are explored and evaluated in the context of the individual’s values and symptoms. Our trained staff is sensitive to the resident’s choices and decisions regarding the course of care.
Unique physical, emotional, spiritual, social and financial needs occur during the final phases of life-limiting illness. Cobble Hill’s Palliative Care Team is comprised of experienced, skilled specialists who work with the attending physician and family to develop care which meets the resident/family needs and wishes. The focus is on caring, not curing.

Cobble Hill Health Center’s Palliative Care Services provide medical care and emotional and spiritual support to people and their families who face life-limiting illness.
At any time during a life-limiting illness, when curative treatments are no longer effective, palliative care treatments for comfort care are usually indicated.
Cobble Hill’s palliative care philosophy recognizes that our residents have come to consider this facility their home and our staff part of their extended family. Our innovative program allows terminally ill residents to continue to live in a familiar environment while receiving additional services.

How Do We Do it?
For over a decade, the Cobble Hill Health Center has led the way in caring for individuals with Alzheimer’s disease. Our comprehensive approach includes program development, research, training, community-based care and caregiver support. We have received the “Best Practices” award and have been recognized by numerous state and national associations. Staff members are specially trained to ensure residents’ comfort and offer increased support to family and friends as the disease progresses, through our palliative care program.
People with Alzheimer’s disease benefit from stimulation and individualized care. At the Cobble Hill Health Center, we address the varied needs and interests of residents with Alzheimer’s disease by offering a range of programs. There is no cure for this chronically disabling disease – but people can continue to enjoy life that has meaning at Cobble Hill Health Center.
Residents are engaged in conversation, participate in small-group programs. On a typical day, residents assist in preparing meals, listen to music or play board games. Specially trained staff are available all day interacting with residents and meeting the participants’ diverse social, physical and emotional needs.
The best program is only as good as the staff that makes it work. At Cobble Hill, our interdisciplinary staff is professional and compassionate. They are committed to delivering excellent care, and they do! Many of our nurses and nursing assistants have worked at Cobble Hill for over 20 years which fosters a team effort. They know each resident, and how that resident changes over the months and years; it’s reflected in their personalized approach. Increasingly diverse cultures require that staff members speak more than one language, (Spanish, French, Russian, Romanian) and are familiar with a wide range of customs, cuisines and continents.
